Community Support Specialist (p/t)

POSITION SUMMARY:

Under the direct supervision of the Program Director for the Marshall Heights Family Success Center, the Community Support Specialist (CSC) is responsible for providing navigation services by connecting residents with District government agencies and other community-based organizations, including collaborative services when needed. The incumbent will assist clients by providing on-sight support and programming to all walk-in residents and conducting outreach activities to promote the activities of the Success Center. The incumbent provides administrative support for the Family Success Center as assigned, which may include, compliance reporting, vendor files, maintaining calendars, answering phones, sending emails, assisting with correspondence, maintaining logs and spreadsheets, and other administrative matters. The Community Support Specialist will display a compassionate nature, be knowledgeable about underserved populations, and provide exceptional customer services to internal and external stakeholders.

DUTIES:

The Community Support Specialist (CSC) will:

  • Works closely with residents to connect them to resources and supports. Referrals for services will be made withing 48 hours. The MHFSC staff will close the referral once the resident’s connection for services has been accepted for intake. They will periodically check with the referral organization for updates and will refer to other organizations if services are not immediately available. The staff will reach out by phone to the referral organization to determine if they are accepting referrals prior to making the referral.
  • Provide ongoing support to all walk-in residents, such as food, clothing, toiletries, rental and/or utility assistance. The CSC will also identify and provide Programming that supports family, youth and senior development.
  • Maintains program activity and outreach records and files as required. Consults with community members and center members to discuss their needs and educate them on East River services for which they can engage to improve their quality of life.
  • Assists programmatic and outreach operations with various administrative responsibilities including research, reporting, meeting coordination, calendar maintenance special event/project planning, and personnel coordination.
  • Other duties as assigned.
QUALIFICATIONS:

A. Education/Work Experience:

Bachelor’s degree in social work preferred; or two to four years of experience working in a social service setting is required; person with lived experience is a plus.
OR
Any equivalent combination of education, training, and experience demonstrates the ability to perform the essential functions of the job. Spanish speaking is highly desired.

B. Certifications/Licensure

Valid Driver’s License required.

C. Computer Skills

Must be proficient in basic MS Office programs including MSWord, Excel, and Outlook. Knowledge of data entry and report generation is helpful.

Community Support Worker

POSITION SUMMARY:

The incumbent will assist clients by providing on-sight support and programming to all walk-in residents and conducting outreach activities to promote the activities of the Success Center.

The FSW will display a compassionate nature, be knowledgeable about underserved populations, and provide exceptional customer services to internal and external stakeholders.

PRINCIPAL ACCOUNTABILITIES:
  • Engage families and other supports to assess family’s needs and implement interventions that are goal oriented and achievable. Builds on the strength of the family to assist them in meeting their needs and respond to crisis and stress appropriately. Coordinates and facilitates the provision of family support services to increase life skills. Report incidents of child abuse and neglect and assist the family in modifying behaviors and/or conditions that contribute to abuse and neglect as necessary.
  • Establishes and maintains healthy, professional relationships to assist families in becoming independent of the child welfare system. Develops strategies to build trust and rapport with families and uses it as a basis in creating a plan to achieve goals.
  • Collects relevant and accurate information required for reporting to assist in the case management function; Documents detailed client contact records to assist in the compilation of a family history and to document progress or lack thereof towards established goals. Completes all necessary documentation and enters required and relevant information into the Efforts To Outcome (ETO) system/database in a timely manner. Regularly assists in analyzing the collected information and actively participates in team meetings that focus on family assessment, family development planning and service agreement development.
  • Regularly maintains relationships with other community agencies, educational institutions, businesses and service providers to advocate for clients. Regularly integrates/links families to informal and formal resources within their community or neighborhood to create natural support systems. Regularly participate in internal and external meetings in spirit of mutual support of families and advocate for needed resources and services and to ensure families are provided with all possible resources available.
  • Assist in the development of a plan for social services for Ward 7 residents. Attend community/neighborhood meetings and events to keep abreast of new and/or changing programs, projects and community resources. Regularly maintain relations with other community agencies, educational institutions, businesses and service
    providers to advocate for clients and needed resources. Serve as a representative of the collaborative at community-based events and/or meetings.
  • Other duties as assigned.

Knowledge of and the ability to follow confidentiality protocols; Ability to organize and prioritize multiple tasks.

SPECIAL CONSIDERATIONS:

A. Working Environment:

Indoor office environment 30% of the time; in the Ward 7 community and other agencies and organizations 70% of the time.

Service Coordinator

The East River Family Strengthening Collaborative (ERFSC) is a non-profit organization whose mission is to empower families, youth, seniors, and communities to become more self-sufficient through integrated and collaborative community-based services utilizing evidence-based practices that are family focused and person- centered.

Job Summary

Service Coordinator will coordinate and implement the Individual Socialization, Counseling, and Information and Referral Assistance services for participants for community residents aged 60+. The Service Coordinator will also travel to the various community dining sites and senior locations in the ward and provide informational sessions to participants. The Service Coordinator is also responsible for providing these services to isolated homebound seniors, which may include visits to the home.

Knowledge, Skills, and Abilities

Knowledge of the needs and problems of the aging; Knowledge of senior services, socialization, community partnerships and outreach; Able to learn and work professionally with others. Knowledge of customer service; Knowledge of issues facing seniors in the community; Knowledge of and the ability to follow confidentiality protocols; Able to proactively solve problems; Able to work with community residents who may be need additional help with navigating programmatic processes; Able to remain patient; Able to exercise good judgment in evaluating situations and making decisions; Able to define and evaluate problems and determine or suggest solutions; Able to organize and prioritize multiple tasks; Able to follow East River policies and procedures.

Education and Experience

Master’s degree in social work preferred. Two to four years of experience in social work and/or case management preferred. Knowledge of information and referral best practices is required. Prior experience working in East River senior services preferred.

Licenses and Certifications Valid Driver’s License require; LICSW preferred but will consider MSW with at least two years of experience or MSW with LGSW

Salary

$65,000 annually
